(AGE) has <br /> prepared this report for the destruction of eight (8) groundwater monitoring wells and <br /> five (5) soil-vapor wells at 104 West Beverly Place, Tracy, California (Site). Additionally, <br /> soil and water (Investigation derived waste) generated from well destruction activities <br /> and previous groundwater monitoring events were removed from the site. The site and <br /> the surrounding area are illustrated in Figure 1. Structures, soil borings, and destroyed <br /> well locations are illustrated in Figure 2. <br /> INTRODUCTION <br /> A total of eight (8) groundwater monitoring wells (MW-1, MW-2, MW-3, MW-4, MW-5, <br /> MW-6A/B, MW-7, and MW-8) and five (5) soil-vapor wells (V-1 through V-5) had been <br /> installed on-site and off-site as part of the subsurface investigation and remediation of <br /> the former underground storage tank (UST) area at 104 West Beverly Place, Tracy, <br /> California. The total depths and screen intervals of the wells are summarized in Table 1. <br /> The wells were destroyed as outlined in the AGE-prepared, Well Destruction Work Plan, <br /> dated 07 October 2015. All wells were destroyed under San Joaquin County <br /> Environmental Health Department (EHD) boring permits (Appendix A). <br /> PROCEDURES <br /> Well destruction activities were performed on 05 January and 02 February 2016 by All <br /> Well Abandonment of Sacramento, California; on 05 January 2016, wells MW-3, MW-4, <br /> MW-5, MW-6A/B, MW-7, and MW-8 were destroyed; and on 02 February 2016, wells <br /> 837 Shaw Road• Stockton, CA 95215 <br /> Telephone(209)467-1006 • Fax(209)467-1118 <br />
