SANJ O A Q U I N Environmental Health Department <br /> -COUN I Y— <br /> Complaint Record Daily Time and Activity Summary from 8/7/2017 to <br /> 8/8/2017 for C00043948 located at <br /> 129 S GRANT ST <br /> Activity Date <br /> Recorded by <br /> 08/07/2017 Received a complaint investigation form from Garrett Backus on 8/4/17 regarding PE 2546 FLORIDO <br /> 30 min the Goodwill at 129 S. Grant in Stockton, which reportedly has a baler leaking fluid SC 004 <br /> onto the ground, located in the back of a warehouse. The complainant believes <br /> conditions are unsafe to work in, and reported poor working conditions. <br /> I arrived on site at 10:00 AM, was escorted by Tim Hernandez, Goodwill staff <br /> member, to the baler in the back warehouse. Met with Steve Celaya, Vice President <br /> of Goodwill, who indicated that he plugged the line in the baler that was leaking at <br /> the time of the incident. He indicated that less than one gallon of hydraulic oil <br /> leaked and that he cleaned the spill using rags. He stated the rags were thrown in <br /> the garbage and were unrecoverable. A small area of concrete(-4 inches in <br /> diameter), immediately adjacent to the baler) had a visible fluid stain, however the <br /> baler was not leaking at the time of inspection. I notified Steve that the <br /> contaminated rags should have been kept until a hazardous waste determination <br /> was made, and then properly disposed of depending on the determination. I <br /> indicated I was going to write and email him an abatement notice. Steve provided <br /> his contact information: (209-466-2311 ext. 1064) (cell: 209-403-1978) <br /> (scel aya@goodwi I l-sjv.org). <br /> 08/08/2017 Wrote an Official Inspection Report, rather than an abatement notice, and emailed PE 2546 FLORIDO <br /> 60 min the report to Steve Celaya (scelaya@goodwillOsjv.org) SC 007 <br /> Created a photo log. <br /> Filed the complainant's workplace complaint with a Cal/OSHA inspector over the <br /> phone (209) 545-7310. <br /> Completed narrative. <br /> Submitted complaint documentation to Garrett Backus for review. <br /> Page 1 of 1 <br />
