PLAN REVIEW AND ARATING PERMIT FEES a. New facility plan review fee of $712.00 (8 hour minimum per facility), $500.00 facility fee (includes one tank), $125.00 per tank after the first tank, plus $8.00 state surcharge fee per tank is due at time of plan submittal plus $10.00 per site state surcharge. b. New installation of an additional tank at an existing facility is $712.00 plan review fee (8 hour minimum per facility) plus $500.00 facility (includes one tank), $125 per new tank after first tank, and $8.00 per new tank state surcharge fee. C. It should be noted that a $712.00 plan review check fee required for each submittal allows for up to eight (8) hours of staff review and/or inspection time. Additional time spent on plan review, meetings, inspections, phone or office consultations will be billed to the applicant at a rate of $89.00 per hour. d. The maximum review time for UST installation plans is 40 working days. A preliminary review will be conducted by the PHS-EHD within 20 working days of submittal date to identify any gross plan deficiencies. e. If gross deficiencies are identified, the plans will be returned with a checklist identifying plan deficiencies. To expedite the review of installation plans, time and a half per hour overtime fee may be submitted to have review completed within 10 working days of submittal date if staff is available to perform the review. 3. PLAN REVIEW GUIDELINES a. The enclosed plan review sheet is to be completed and submitted with the three sets of drawings to PHS-EHD. b. A copy of the California UST Regulations may be obtained from the California State Water Resources Control Board at (916) 227-4303. C. Permits and/or plan check approval may also be required by the San Joaquin Valley Unified Air Pollution Control District, the County/City Building Department and the Fire District in your respective area. d. A minimum of 3 inspections will be conducted on the UST system installation. A $89 per hour inspection fee shall be charged for additional inspections or reinspection as required. (See required inspections below) e. Applicant/contractor to adhere to the approval plan and any change or deviation shall be reported to and approved by PHS-EHD prior to installation. The application for installation of underground tanks is only valid for the calendar year in which it has been issued. A permit may be extended into the next calendar year if a letter is sent to PHS-EHD requesting this extension thirty days prior to the end of the calendar year. A one time, one year extension may be granted by PHS-EHD upon receipt of this letter.
