San Joaquin County <br /> t Environmental Health Department <br /> ram <br /> Complaint Record Daily Time and Activity Summary from 3/1/2015 to <br /> q'..t,.v. 7 6/17/2020 for C00039923 located at <br /> 17100 S HARLAN RD <br /> Activity Date Recorded by <br /> 06/24/2015 Called the Michael Parassi to obtain information on the spill. He stated that about 2 PE 2546 VO <br /> gallons of adhesive was spilled that could have been due to a faulty spray nozzle. SC 544 <br /> He stated that the clean up is in progress and is half way through. <br /> Called Michael back to check the status of the clean up and let him know that I will <br /> be coming out to the facility. <br /> Arrived onsite to witness the clean up. Met Michael from Carpenter Co onsite. He <br /> showed me where the spill occurred. He stated that since it is adhesive, they are <br /> having a tough time cleaning it. Clean up was about 75% completed. Observed a <br /> open 55 gallon drum with overflowing waste on site next to the spill. The 55 gallon <br /> drum contained the adhesive, cardboard and gloves. Michael stated that cardboard <br /> was used by night crew member to minimize the spill from the night before. Directed <br /> Michael to containerize the waste in a closed container, move some waste into <br /> another compatible container since the 55-gallon was too full and label it while it is <br /> pending analysis. <br /> 06/25/2015 Called Michael to follow up on a spill. He was not able to answer the phone due to a PE 2546 VO <br /> meeting. Asked the receptionist for his email address, SC 544 <br /> michael.parissa@carpenter.com. <br /> Emailed him to see the status of the clean up and his hazardous determination. <br /> 07/03/2015 Documented photos and activity in the ER log. Printed all emails from Michael PE 2546 VO <br /> Parissi and reviewed today. SC 544 <br /> 1) Received MSDS from Michael through fax on June 24, 2015 for adhesive <br /> 2) Received an email on June 25, 2015 from Michael about status of the clean up. <br /> In the email, Michael mentioned that there is a delay in the clean up due to finding <br /> another 55 gallon drum and that it is classifying the waste as hazardous. <br /> 3) Received photos of the spill area through email on June 30, 2015 from Michael. <br /> 4) Received photos of the 2-55 gallon drum closed and labeled hazardous waste. <br /> Emailed Michael to send disposal records to EHD once he receives a copy. <br /> 07/03/2015 Created Proposition 65 notification. PE 2546 VO <br /> SC 203 <br /> 07/20/2015 Received a manifest from Michael for the spill (item #3) on July 15. Confirmed that PE 2546 VO <br /> item#3 is waste stream from the MSDS provided and it is. Still awaiting for a copy SC 544 <br /> of the final manifest with TDSF signed off. <br /> 08/24/2015 Called Mike P. to request for final copy of disposal record. He said he will get back PE 2546 VO <br /> to me. SC 544 <br /> Mike emailed back with the final signed off copy. Abated complaint. <br /> Page 1 of 1 <br />
