Box 2009 <br /> Stockton, California 95201 <br /> Reference: Phase II Environmental Investigation at 804 and 810 East Black <br /> Diamond Way in Lodi, California, Versar Project No. 3301-002 <br /> Dear Ms. Turkatte: <br /> Please find enclosed one completed permit application and a check for the $323.00 to <br /> cover the necessary fees for one soil boring (Attachment I)to be drilled at 804 and 810 East <br /> Black Diamond Way in Lodi, California(Site). Please note that this soil boring is being drilled <br /> for the sole purpose of assessing whether an identified up gradient leaking underground fuel <br /> storage tank site at the Parmar Texaco has impacted the soils and/or groundwater beneath the <br /> Site. There is no known petroleum hydrocarbon contamination at the Site, nor has there ever <br /> been any petroleum hydrocarbons sources. Versar,Inc. (Versar) is submitting this permit on <br /> behalf of McDonough, Holland& Allen and with the permission of the Site owner, Mr. John <br /> Donovan. Included in Attachment II is an Agreement for Right of Entry between McDonough, <br /> Holland&Allen's client Reliance Insurance and Mr. Donovan. <br /> The Site location is shown in Figure 1 and the location of the proposed boring can be <br /> seen in Figure 2. The boring will be drilled to a depth of approximately 65 feet below ground <br /> surface (bgs), based on a depth to groundwater of 60 feet. As drilling progresses, soil samples <br /> will be collected to determine lithological and environmental conditions. Soil samples will be <br /> collected at five-foot intervals beginning at five feet bgs using a California-modified split-spoon <br /> sampler fitted with brass sleeves. Based on field screening using a photo-ionization detector,two <br /> soil samples will be submitted for laboratory analyses. The boring will be extended into <br /> groundwater to allow a"grab" groundwater sample to be collected. All soil and groundwater <br /> samples will be analyzed for total petroleum hydrocarbons as gasoline (TPH-G) and benzene, <br /> toluene, ethylbenzene, and xylenes (BTE3). After groundwater sampling,the boring will be <br /> backfilled with a cement grout with less than 5 percent bentonite tremied from the bottom of the <br /> boring to five feet above groundwater. The boring will be backfilled with drill cuttings <br /> augmented by Portland cement, if necessary, from the grout seal to surface level. Any additional <br /> drilling spoils will be spread on site.
