Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediate.Noncompliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION --TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Spoke with the Chief Executive Officer and the Warehouse Manager regarding the complaint. They state that Area Wide Pest <br /> Control conducts monthly service and the facility conducts weekly checks of the glue board boxes. <br /> Observations this date: <br /> 1)There are 16 glue board boxes positioned throughout the interior of the warehouse and 10 bait boxes positioned around the <br /> exterior of the warehouse. <br /> 2) Three mice, rodent droppings and insects were observed on some of the interior glue boards. <br /> 3) Fresh and dry rodent droppings were observed throughout the interior of the warehouse perimeter with heavy accumulation <br /> at the southeast corner(behind Cooler#1 -storage cooler). <br /> 4) Fresh and dry rodent droppings were observed at cereal, popcorn, and pet food pallets. <br /> 5) The southwest corner of the warehouse (behind the shopping freezer)has a strong urine odor. <br /> Per Warehouse Manager, pallets are sorted when received. These salvage grocery pallets come from neighborhood markets <br /> and big box stores. Any old, expired, open, or otherwise compromised products are removed and discarded. <br /> Corrective Action required: <br /> 1 )Increase pest control frequency. <br /> 2) Remove all glue boards that have vectors. Replace with new glue boards and monitor for new activity. <br /> 3) Position snap traps in heavy activity areas and monitor daily. <br /> 4) Sweep up all rodent droppings and monitor areas daily for new activity. <br /> 5) Call Area Wide and request a service visit for this week.
