Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodbome illness.All major violations must be corrected immediate/v.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION --TEMP°F COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> This is a re-inspection from the complaint inspection conducted 7-18-17. <br /> Observations this date <br /> 1) Observed a dead rat in a snap trap <br /> 2) Observed rodent droppings throughout the facility interior perimeter. <br /> 3) Glue boards and snap traps are in place throughout the facility <br /> Corrective action: <br /> 1) Continue daily checks of glue boards. <br /> 2)An increase in pest control frequency is recommended (from monthly to bi-weekly) until the issue is reduced. <br /> 3) E-mail the most recent invoice from Area Wide to Kadeanne Linhares (klinhares@sjcehd.com) <br /> 4) Reset snap traps when "sprung"...position along the perimeter of the building's interior. <br /> The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br /> Safety Code.
