A permit application with an $89 fee to install the additional wells <br /> should also be submitted, as Vroposed in the PAR. The monitoring well recently covered with anew asphalt <br /> layer is to be located, repaired as necessary, and sampled immediately. An update to the site map shall be <br /> submitted and PHS/EHD r-.uests that the "B" designations of wells be dropped in an attempt to avoid <br /> confusion. The wells whit are currently hooked up to the vapor extraction system should be clearly <br /> presented. Quarterly monkar�ng reports for first and second quarter, 1994 are to be submitted to PHS/EHD <br /> immediately and future reports shall be submitted prior to the next quarter's monitoring event. A complete <br /> "zero" line for both soil and zroundwater has not been delineated. Proposals to complete this requirement <br /> shall be included within a?-AR addendum along with a time schedule for their timely implementation. <br /> 441 W. Future quarterly reports shall be submitted prior to the next <br /> quarter's monitoring. An adendum to the PAR shall be submitted and will include a discussion and a <br /> feasibility study for an add conal alternative remediation proposal that would restore the groundwater to <br /> beneficial use status. Furth=discussions shall be scheduled to consider the possible off site and upgradient <br /> source(s) of groundwater c—m—u mination and shall include other responsible parties, as well as PHS/EHD <br /> and Arco representatives. <br /> 800 E. Kettleman, Lodi ARCO Facility 2076 <br /> PHD/EHD contact - Mary Mears <br /> The groundwater sample wits of the "Fourth Quarter Monitoring Report" evidenced groundwater <br /> contamination. Kyle Chris_indicated that there was some problems with the decontamination procedures <br /> which were being used by sampling firm. There are no plans to begin soil remediation within the next <br /> year; however, the groundwater will continued to be monitored and sampled quarterly. <br /> 1612 Hammer Lane, Stockton ARCO Facility #548 <br /> PHS/EHD contact - Mary Meays <br /> The contents of the PAR w= discussed and the addendum which was requested by PHS/EHD was <br /> submitted at the meeting. <br /> The contamination which was detected in MW6, during the first quarter 1994, may have been a result of <br /> the decontamination prods used because numerous sites which had never evidenced groundwater <br /> contamination had detectaEe contamination during this sampling event. PHS/EHD was informed that no <br /> contamination was detected in the samples collected during the second quarter monitoring event, thus <br /> indicating that the groundwater plume has been defined. <br />
