SAN10AQ U I N Environmental Health Department <br /> COUNTY— <br /> Complaint Record Daily Time and Activity Summary from 1/8/2017 to <br /> 1 /3/2017 for CO0044224 located at <br /> 2941 NAVY DR <br /> Activit Date Recorded by <br /> 09/08/2017 9:30 AM- Received a notification regarding a release of 64 gallons of ethanol into PE 2546 FLORIDO <br /> 150 min a concrete containment with up to 5-10 gallons released onto the ground and into SC 544 <br /> the soil caused by a drain valve being left open on a pump. No waterways or storm <br /> drains were impacted. Caller reports booms and absorbents have been deployed. <br /> 10:40 AM-Arrived on site. NuStar Energy personnel on site and conducted spill <br /> clean-up. No staining of ethanol was observed a the time of inspection. Six drums <br /> of excavated contaminated soil and absorbent materials were generated. No <br /> waterways/storm drains were impacted. Clean Harbors to pick up drums. <br /> Clean-up is complete. Wrote the Notice to Abate and provided the notice to Ray <br /> Smith, NuStar Energy Operations Supervisor. Left site at 11:30 AM. <br /> Completed Emergency Response Record. Emailed the Notice To Abate to Ray <br /> Smith (ray.smith@nustarenergy.com) and Daniel Thomas <br /> (daniel.thomas@nustarenergy.com) of NuStar Energy. <br /> Completed narrative. <br /> 09/08/2017 Prop 65 completed PE 2546 FLORIDO <br /> 30 min SC 203 <br /> 09/28/2017 Called Ray Smith and left him a message reminding him to send me the final PE 2546 FLORIDO <br /> s min hazardous waste manifest within 30 days of the spill date, 9/8/17. Smith returned SC 544 <br /> my call stating that Clean Harbors is scheduled to pick up the waste tomorrow and <br /> Smith would email the manifest to me by Monday morning 9/2 at the latest. <br /> 09/29/2017 Received an email from Ray Smith with an attached initial hazardous waste PE 2546 FLORIDO <br /> s min manifest. He indicated that he will email me the final signed copy once he receives SC 544 <br /> it. I replied indicating that i would be in contact with him around 10/8/17 unless I <br /> receive the manifest by then, and that all of my time spent requesting the manifest <br /> is billable. <br /> 11/03/2017 Created and printed photo log. Gathered complaint documentation and provided to PE 2546 FLORIDO <br /> 42 min Garrett for review. SC 544 <br /> Page 1 of 1 <br />
