LODI 95240 <br /> Owner/Operator: LAXMI US 1234 FOOD CORP Telephone: (209) 369-1361 <br /> Program Element 1600 - FOOD PROGRAM I Complaint#: C00043059 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT ALLEGES THEY WERE AT THE FACILITY ON 3/7/2017 ATAPPROXIMATELY 7:30 AM AND <br /> OBSERVED THE CUSTOMERS AT THE TABLE NEXT TO THEM CONSUMING PANCAKES. WHEN THE CUSTOMERS <br /> PICKED UP THE SYRUP THAT IS STORED ON THE TABLE, TWO BABY COCKROACHES RAN OUT. COMPLAINANT <br /> STATED THAT THEY COMPLAINED TO THE WAITRESS WHO THEY CLAIM ACTED LIKE IT WAS NO BIG DEAL AND <br /> WALKED AWAY PLEASE CALL COMPLAINANT WITH FINDINGS. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness. All major violations must be cofrected immediately. Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM --LOCATION --TEMP ° F --COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> I spoke to the person in charge and manager who were not aware of the complaint prior to my visit. <br /> Pest control services facility monthly and as needed. Most recent service was last month. <br /> I observed 6-8 syrup holders at tables and did not observe cockroaches at this time. <br /> Facility to continue/increase effective pest control as needed. <br /> The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br /> Safety Code.
