SANJ O A Q U I N Environmental Health Department <br /> COUNTY— <br /> Complaint Record Daily Time and Activity Summary from 8/1/2014 to <br /> 8/1/2017 for C00042071 located at <br /> Lower Sacramento Road 1/2 mile North of Woodbridge <br /> Road <br /> Activity Date <br /> Recorded by <br /> 08/15/2016 EHD response to a PG&E high pressure natural gas transmission line leak caused PE 2546 LOPEZ <br /> 210 min by a vehicle accident; checked surrounding area 1/2 mile away and down wind; SC 904 <br /> 08/15/2016 8.00 pm: I responded to a natural gas release from a 10" high pressure gas line PE 2546 NAIDU <br /> 240 min with a 3/4 inch hole due to a car accident. I met with Mike Raab (PG&E)209 607 SC 944 <br /> 4691. He said that PG&E is in the process of closing a section of the pipe to <br /> isolate the leak. Once the pipe has been isolated and de-pressurized it will be <br /> repaired. I called Woodbridge Fire Department and was told that they were there <br /> for the vehicle accident and left the scene to PG&E to mitigate. He said that they <br /> went to the two house in the proximity and one was empty. THe occupant of the <br /> other house evacuated voluntarily. <br /> Robert Lopez and I went around the area with the 5 gas meter to see if the release <br /> was a concern. we did not get any hit. We knocked and talked to the resident half <br /> mile east of the incident and the had no idea what had happened. I told the to call <br /> the Woodbridge Fire if they smell gas. <br /> 08/16/2016 Return air monitoring equipment to the EHD hazmat bay. <br /> 60 min PE 2546 LOPEZ <br /> SC 004 <br /> Page 1 of 1 <br />
