G San Joaquin County Environmental Health Department R" F Complaint Record Daily Time and Activity Summary from 3/1/2015 to 6/17/2020 for COGP1Z9SB located at Super Store Industries - Grocery Division 16888 McKinley Ave, Lathrop Activity Date Recorded by 10/24/2015 At 0130 the Sheriff office notified me of 5-7 gallons of Diesel spill at Super Store in PE 2546 SAEED Lathrop. I was told that the RP will do the clean up and no water ways involved SC 944 and nothing went into the storm drains. I was given Mike Hyllen, RP at the site. I called Mike Twice and he did not answer. I left voice mail for a call back. I paged the ER page. I called the Sheriff again and asked him if he has Mike Cell number or if he was given a different number, and he told me he does not have more information. I searched in CERS for the RP 24 hours Primary Emergency contact person, I called the contact person and he did not answer. I notified Jamie DeLaRosa of the incident. Garrett called me and I notified him of the incident. Garret told me that he will go to inspect the spill in the morning, and no need to respond now. Later, the sheriff called and told me that he was able to get hold of mike and asked if I need mike to call me. I told the sheriff that I talked with Garrett and he will go to the site in the morning time to inspect, sheriff told me he will let mike know. 10/24/2015 9 am-12 noon. Spill report 15-6290, Superstore, 2400 E Louise Ave/16888 PE 2546 BACKUS McKinley, 7 gallon diesel spill from refrigerator truck diesel tank (collapsed trailer) SC 974 at terminal. Responded to site to verify cleanup and met Shawn Fraser, manager, on site. Diesel spilled on asphalt in front of the warehouse offices. Storm drain not impacted. Site personnel performed dry cleanup last night. Spill waste stored in one 55 gallon drum, labeled as HVV and stored in the maintenance area at the south west side of the warehouse. Facility has HVV generator permit with the EHD. 10/24/2015 1:48 am- 2.06 am. Spill report 15-6290, Superstore, 2400 E Louise Ave/ 16888 PE 2546 BACKUS McKinley, 7 gallon diesel spill from refrigerator truck diesel tank (collapsed trailer) SC 944 at terminal. Terminal personnel conducted cleanup. Storm drain system not impacted.
