Effective April 6, 2007, Plymouth Place has adopted the following protocol when the un-named _ <br /> odor/irritanf.is determined to be present in the facility:- <br /> If the odor/irritant'occurs between Monday-Friday, 8:30 a.m.-5:00 p.m., facility ' <br /> staff will immediately notify the J.H. Simpson company by calling(209) 466-1477. <br /> If the odorArritant is noticed at any time other than Monday-Friday, 8:30 a.m.-4:30 p.m., <br /> the residents, or their family members, or significant others will immediately notify the <br /> J.H. Simpson company by calling (209) 466-1.477. <br /> Do Not Call 94'4 <br /> - <br /> I. 'Notify JR. Simpson our HVAC vendor by calling: (209) 466-1477 <br /> This contact is to be made by staff if the odor/irritant is discovered Monday-Friday,- <br /> between 8:30 a.m. and-4:00 p.m. <br /> 4T the odor/irritant is discovered at other days or times, the notification will be made <br /> v"by facility residents, their:family members, or significant others. <br /> 2. _.5tafron duty will tape plastic sheeting in the hallway to separate the office areas and <br /> z Ahe south hall from the remaining facility areas on the first floor. <br />- _ -The plastic sheeting and duct tape will be kept at the reception desk. <br /> If staff is not on duty, the J.H. Simpson technician will be'responsible for taping . <br /> plastic sheeting in the hallway.o <br /> - - <br /> 3. _ -If staff is present, they will lock up the business offices. <br /> 4". :Staff(and any residents, visitors;or their guests) will evacuate'the immediate area T <br /> involved in the odor/irritant discovery: Staff is to remain near the building, and return - <br /> W work when informed the area has been cleared. <br /> 5. A J.H. Simpson technician will come to the facility within 2 hours to make the <br /> following inspection: - <br /> -Ai Check all ventilation equipment to assure that it is intact and functioning properly. <br /> - Bk Secure the immediate area where the odor/irritant has been identified. <br /> C Locate the-HVAC unit that serves the affected area. - <br /> D' Open the unit panels. <br /> E Use the Carbon Monoxide meter to measure readings. <br /> 17 , Open available doors and/or windows to promote affected area ventilation. <br /> 6. J.H. :Simpson staff will notify facility management of their response and will <br /> ;provide'a written report of their activities and recommendations. <br />
