Web: www.co.san- 'oa in,ca.us/ehd <br /> } <br /> �9�•_.....NSP <br /> EMERGENCY RESPONSE INVESTIGATION REPORT(INCLUDE CHRONOLOGICAL HISTORY,RECOMMENDATIONS,CLEANUP end FINAL DISPOSITION): <br /> ff2OO7 <br /> Time and Comments Staff <br /> Name <br /> pm: Received a message that another incident had occurred at Plymouth Place and that the Stockton Linda <br /> Department had responded, had closed off the common area on the first floor and was requesting EHD Turkatte <br /> ance in the clearance of the affected areas. <br /> 4:46 pm: I arrived onsite. The Stockton Fire Department had already left the scene. I observed that the <br /> front door area was taped off with caution tape. I spoke with Rosanne Martin, resident and night manager <br /> for the facility. She stated that the Fire Department closed off all the common areas on the first floor and <br /> advised her that they can reopen only after EHD has given clearance. 1 advised Ms. Martin that the EHD <br /> would need a clearance by a Certified Industrial Hygienist. She stated that I would need to speak to Paulette <br /> Rivera,the manager of the building and she gave me her phone number(916) 501-1186. 1 wrote a notice to <br /> consult with a Certified Industrial Hygienist to determine source of problem and to abate source. The notice <br /> was received by Ms. Martin and she stated she would forward to Ms. Rivera. At that time Ms. Rivera drove <br /> up and I explained to her that the EHD would require a Certified Industrial Hygienist to certify the area is <br /> acceptable for occupancy and she said she would relay the information to her boss, Bruce Burke. She spoke <br /> with Bruce Burke on the phone and he was upset. She gave me the phone. Mr. Burke was very upset and <br /> stated that they had already spent $8500 on a Certified Industrial Hygienist the first time this happened and <br /> that by the time they get there the chemical has dissipated. I stated that the EHD requires the area be <br /> examined by a Certified Industrial Hygienist and that they give the clearance for re-occupancy. He stated <br /> that they did not want to spend another $8500. 1 advised Mr. Burke to then work with the Stockton Fire <br /> Department directly to re-open the area as they were the agency who closed the area off. Mr. Burke asked <br /> me to talk with his boss in Long Beach, Stewart Hartman (951) 347-5541 cell/(562) 257-5100 x5109. <br /> stated 1 would. <br /> 5:30 pm: Called Mr. Hartman at office number and left a message. <br /> 5:45 m: Left site. <br /> March 8:15 am: Called and spoke to Stewart Hartman. He relayed all the steps they have taken so far to abate Linda <br /> 30,2007 recurring incidents at the facility (including hiring the Certified Industrial Hygienist after the first incident, Turkatte <br /> looking at video tapes,working with the police department,hiring an HVAC service company). He stated <br /> they believe a resident,friend or relative of a resident,or a member of the staff is responsible for these <br /> incidents and they feel they are doing everything they can. Mr. Hartman was also upset that the fire <br /> department had closed off the common area and that EHD was asking for a Certified Industrial Hygienist to <br /> be hired again. I advised Mr. Hartman that when an incident of this nature occurs and there is no response <br /> by his management for abatement, that they call the fire department and EHD. I stated that it is the EHD's <br /> protocol to have a Certified Industrial Hygienist come in to test and confirm area is suitable for occupancy. i <br /> advised Mr. Hartman to develop his own response plan that his staff can implement on their own in the <br /> event of another incident. The plan could call for the area to be evacuated and a private response firm to be <br /> called to respond and abate the area. This would negate the need for the residents to call the fire <br /> department and the EHD. He stated he had called a Certified Industrial Hygienist and he is to visit the facility <br /> today and he would let me know the status. Called Stockton Fire Department and requested copy of report. <br /> 2:20 pm: The front counter area received a call asking my name and title but no name or message. I called <br /> Stewart Hartman and he stated he did not call me. Called Paulette Rivera and left a message to call front <br /> counter and have me paged if I was needed (my phone down for move). <br /> 4:30 pm: Paulette Rivera paged me. Called her back. The Certified Industrial Hygienist had prepared a <br /> letter report of his inspection of the facility. She read the letter which recommended the area closed off by <br /> the fire department to be re-opened. 1 told her she could open the area and, because no fax machines were <br /> operational due to our move. I would stop by after work to pick up copy of letter. <br /> 5:20 pm: Arrived at Plymouth Place and received a copy of the letter from Richard Beall of Entek <br /> Consulting Group. While in the building I walked through the hall and common gathering area. I did not <br /> observe any odors and irritants. <br /> ER RECORD MODIFIED Page 3of3 02-05-2007 <br />
