Called Jess Dorfman at PE 2546 MANZO <br /> 90 min 440-349-2700, ext. 306. Who reported that the spill occurred at 2:20 pm, no SC 544 <br /> waterways were impacted, Patriot Environmental were on their way for clean up and <br /> area scheduled to arrive at 5 pm. The billing address for this spill is ERTS,6001 <br /> Cochran Rd, Solon, Ohio, 44139. Approximately %gallon of Clarifloc C-308 spilled <br /> on the pavement due to missing tanker truck wash cap. Someone was bringing a <br /> new cap, the spilled material is non-hazardous per Jess, and she will email the <br /> SDS. Patriot will apply absorbent and ERTS will make a hazardous waste <br /> determination. Researched the material, the SDS and the UN listing number. <br /> Discussed spill with Garrett Backus, lead sr., and Haza Saed who received the <br /> complaint. <br /> 11/07/2016 5:00 pm-after hours 6 MANZO <br /> 254 <br /> PE <br /> 138 min 5:15 pm onsite, met with Steve the truck driver for Quality Carriers (whose truck was E 54 <br /> leaking the liquid), DOT 076600, reviewed the SDS he had, the material is a <br /> 44 <br /> corrosive liquid with fish toxicity <260 mg/kg. Patriot is onsite applying absorbent, <br /> the spill is several feet wide and 40-50 feet long, no storm drain was impacted. 2 x <br /> 55 gallon containers of PPE and waste absorbent were generated. Issued a NTA <br /> asking for waste disposal records, prepared ER record and Prop 65. Clean up was <br /> completed. The two drums (labeled as hazardous waste)were left onsite at the gas <br /> station, Brandon McMaster with Patriot indicated that they will be picked up <br /> tomorrow. Downloaded inspection pictures, created picture file. <br /> 08/01/2017 Reviewed the complaint, to date no disposal information has been received, called PE 2546 MANZO <br /> 30 min Brandon with Patriot environmental 916-317-0144, requested manifest copy, he SC 544 <br /> indicated he will research it and get back to me. Called the RP (ERTS was listed <br /> 440-349-2700), spoke with Tod, who took all the info down and indicated that he will <br /> forward the final disposal shortly. Provided my email address. <br /> 08/11/2017 Reviewed the complaint and the disposal documentation. Reviewed the disposal PE 2546 MANZO <br /> 80 min documentation received from Patriot Environmental, the provided SDS for the spillei SC 544 <br /> material indicates that the spilled polymer is non-toxic and it's pH is 4-8, as such <br /> the waste was classified as non-hazardous. A non-hazardous disposal manifest <br /> was provided for the disposal of two 55 gallon containers of waste debris. <br /> Abated the complaint. <br /> Page 1 of 1 <br />
