AN J O A Q U I N Environmental Health Department <br /> COUNTY— <br /> Complaint Record Daily Time and Activity Summary from 1/5/2017 to <br /> 6/5/2017 for C00043200 located at <br /> 1990 N PICCOLI RD <br /> Activity Date Recorded by <br /> 04/03/2017 Kim Blackwell gave me spill report(CTRL#172535) that was emailed to EHD at PE 2546 SAEED <br /> 180 min 1:25 am today. The incident happened at 1:00 am at Unified Grocers warehouse SC 544 <br /> located at 1990 Piccioli Road in Stockton today. The spill report stated that"a <br /> forklift operator was moving pallet of bleach from an upper level and due to <br /> unknown reason the pallet fell and released approximately 180 gallons of <br /> household bleach. The release was contained to the cement floors and did not <br /> enter the storm drains or waterways. The release will be mitigated by a third party <br /> contractor'. Around 09:08 am EHD received a notification call of the incident from <br /> Unified Grocers. At 09:13 am I called the reported party, Thelma Sass. Thelma <br /> confirmed the information reported on the spill report, and told me that HazMat <br /> Trans(Third party contractors) are coordinating the cleanup and their ETA is <br /> around 11:00 am today. I asked Thelma why it is taking too long to respond since <br /> the incidents happened around 1:00 am. She told me that she called HazMat <br /> Trans right after the incident, but it took them time to coordinate with the cleanup <br /> crew. She added the area was secrure with caution tape and isolate from entry. <br /> Thelma told me there was no exposure, injuries, or evacuation due to this incident. <br /> Also, she confirmed that the spill did not impacted the waterways or storm drains. <br /> I obtained HazMat Trans contact Info (Mike Hammer via 909-889-5607)to follow <br /> up with the cleanup crew ETA. At 10:46 am I called Mike and he told me that he is <br /> the broker, and he hired Double Barrel Environmental out Bakersfield to conduct <br /> the cleanup. I asked Mike about their ETA, and he told me that he will contact <br /> them and get back to me. At 10:52 am, Mike called me back and told me that the <br /> cleanup crew ETA within two hours. I obtained the contact info. For Double Barrel <br /> Environmental ( Chris Parko via 661-203-7645). 1 asked Mike about the time <br /> Unified Grocers notified them of the incident and told me around 1:30 am but it <br /> took sometimes to find a contractor to do the cleanup and get the equipment and <br /> the crew ready. At 12:35 pm I called Chris and told me they are 50 minutes away <br /> from the site. At 1:15 pm I responded to the incident. At 01:30 pm I arrived on <br /> site, and I met with Thelma. Contractors arrived on site at the same time. I <br /> inspected the area where the spill occurred. A pallet full of%:gallons household <br /> bleach containers fell into the floor, and some of the containers were impacted <br /> leaked into the floor. The amount of bleach spilled was unknown. I was given <br /> permission to take photos by Thelma. The spill contaminated three Aisles inside <br /> the warehouse. Dry absorbent was placed to contain the spill. Caution tape was <br /> on place to restrict entry. Chris told us it will take them about 30 minutes to finish <br /> the cleanup. We asked Chris if they can estimate the total bleach leaked into the <br /> floor. I completed the notice to abate, had Thelma sign it and gave her a copy. <br /> Later on we realized that the cleanup would take longer than the contractors <br /> anticipated. Thelma asked the contractors about the estimated time to finish the <br /> cleanup and told us about 05:30 pm today. I told Thelma that I will leave and <br /> someone else will come after 5 pm to witness the final cleanup. The cleanup crew <br /> estimated about 168 gallons spilled into the ground. At 03:14 pm I called Garrett <br /> and updated him with the status of the incident, and told him that I will leave the <br /> site because they are not finishing up before 5 pm. Garrett told me to come back <br /> to the office and discuss it with Elena. Thelma told me she will take photos after <br /> the cleanup and send them to us; I told her it is up to the next ER person to <br /> decide, but most likely will be here to witness the final cleanup. At 03:27 pm I left <br /> the site. At 03:45 pm I was back at the office and I discussed the incident with <br /> Elena. <br /> Page 1 of 2 <br />
