BROADWAY AVE <br /> STOCKTON,CA <br /> APN: 143-150-08& APN: 143-150-22 <br /> Mr.Trame <br /> On March 4, 1999 San Joaquin County Public Health Services Environmental Health Division (PHS/EHD) <br /> received a formal complaint regarding potential improper wastewater disposal at the above referenced site <br /> (APN's 143-150-08, and 143-150-22). The complaint alleges that untreated wastewater is collected and <br /> pumped into three on-site subsurface disposal wells. A copy of a written report with photographs prepared <br /> by the City of Stockton, Department of Municipal Utilities, has been submitted to PHS/EHD and clearly <br /> shows the wastewater system in use at the time of discovery. At the time this complaint was received, this <br /> facility was not connected to public sewer. <br /> PHS/EHD has recently implemented a Local Agency Groundwater Protection/Underground Injection <br /> Control Program in accordance with guidelines prepared by the United States Environmental Protection <br /> Agency(USEPA). The primary focus of the program is the identification,evaluation, and potential closure <br /> of improper disposal wells and identification of other land use practices in San Joaquin County that may <br /> lead to degradation of drinking water supplies. PHS/EHD seeks your cooperation in providing information <br /> that will assist in the evaluation of your facility. <br /> Please describe any on-site wastewater disposal systems both existing and constructed since the time you <br /> began operating at this location. This includes, but is not limited to, construction details of the on-site <br /> disposal wells, septic, and wastewater conveyance systems. Please include copies of any construction <br /> permits or waste discharge permits. <br /> Records of water use indicate that this facility uses large volumes of water on a daily basis. Please describe <br /> in detail the past and present water uses, fluid and sludge waste stream characterization (analyses), and <br /> wastewater disposal methods at this facility including any pre-treatment, recycle, storage, on-site and off- <br /> site disposal with supporting documentation. <br /> This correspondence is issued to both the owners of the property and operators of record for the facility <br /> described above. PHS/EHD requests the facility operator read, complete, sign, and return the enclosed <br /> Master File Record Form (MFR) and submit the required 3 hour minimum advanced fee for services <br /> totaling 5234.00 (Payment to: PHS/EHD). This site is currently classified as an Underground Injection <br /> Control Program Environmental Assessment,assigned to the Site Mitigation Unit at PHS/EHD. <br /> A Division of San Joaquin County Health Care Services <br />
