Broadway <br /> Stockton, CA 95205 <br /> San Joaquin County Public Health Services, Environmental Health <br /> Division (PHS/EHD) , has reviewed the Site Assessment and <br /> Remediation Report for Above Ground Waste Oil Tank Area dated May <br /> 21, 1991. In the report you state soil borings were hand augured <br /> by Environmental Science and Engineering personnel on December 27, <br /> 1990, and that Gregg Drilling and Testing drilled four (4) soil <br /> borings on January 29, 1991, and six (6) more soil borings on <br /> February 22, 1991. Prior to drilling soil borings or monitoring <br /> wells in San Joaquin County, a permit must be secured from this <br /> office by a contractor with a C-57 license. <br /> Any subsurface investigations in which soil borings or monitoring <br /> wells are drilled require the submission of work plans and the <br /> securing of permits from this agency. <br /> Enclosed is a packet from our agency entitled "Site Mitigation <br /> Program - Guidelines For Work Plan and Report Submittal. " The site <br /> at 1700 N. Broadway, Stockton, is in category IV, "Other Government <br /> Agency Oversight Review Process. " As per your phone conversation <br /> with Diane Hinson, Supervising REHS, of our office, on July 10, <br /> 1991, if you had submitted a work plan and permit application, our <br /> agency could have contacted Regional Water Quality Control Board <br /> and obtained approval to issue the required permit from our agency. <br /> Although our agency does not have the authority to comment on the <br /> work performed at 1700 N. Broadway, Stockton, we are responsible <br /> for reviewing the results of the investigation to determine if the <br /> site is to be placed on our contaminated site list and if a <br /> referral to the appropriate state agency is to be made. The <br /> $159. 00 fee for "Other Government Agency Oversight Review" covers <br /> the time our agency spends reviewing reports and completing the <br /> necessary paperwork. <br /> Please submit the completed "Site Mitigation Acknowledgment/Request <br /> for Services Form" and the $159.00 fee within thirty (30) days of <br /> the date of this letter. <br /> A Division of San Joaquin County Health(ace Services C> <br />
