PROVEN TECHNOLOGIES AND REMEDIES GUIDANCE—REMEDIATION OF METALS IN SOIL <br /> Under unusual circumstances the removal action may not be carried out as planned <br /> because conditions not anticipated in the workplan were encountered. Institutional <br /> controls (ICs) or other actions may be required if the cleanup goals cannot be achieved. <br /> 7.8 HEALTH AND SAFETY MONITORING <br /> The workplan should reference the health and safety plan (HASP) that addresses site- <br /> specific excavation and restoration and the health and safety issues that may arise at <br /> the site. These health and safety requirements should be followed by all personnel, <br /> including contractors and subcontractors conducting work at the site. The HASP used <br /> during site characterization activities may be amended to include excavation and <br /> restoration activities. The HASP should be prepared in accordance with the <br /> requirements of California Code of Regulations, title 8, section 5192 and all applicable <br /> federal, state and local laws, ordinances, and regulations and guidelines. <br /> The HASP should at a minimum address the following: <br /> • Identification of activities being carried out, the associated risks and the <br /> measures in place to prevent injury; <br /> • Names and titles of personnel in charge; <br /> • Emergency action plan; <br /> • Location of HASP, a copy should be on site at all times; <br /> • Method utilized to train all personnel on site on HASP and excavation safety <br /> awareness (e.g. tail gate meetings and frequency); <br /> • Method for identifying hazards, documentation and correction of hazards; <br /> • System in place to ensure that all workers comply with the rules to maintain a <br /> safe work environment. ( e.g. disciplinary methods for workers who fail to <br /> comply) <br /> 7.9 COMPLETION REPORT <br /> The workplan should briefl� identify the key elements that will be covered in a <br /> completion of work report' (hereafter referred to as the "report") and the anticipated <br /> date of submittal. The report should be signed by a professional engineer or a <br /> professional geologist licensed in California with expertise in hazardous substance site <br /> cleanup. <br /> An annotated outline for the report is provided in Appendix D5. At a minimum, the <br /> report should provide the following: <br /> • Summary of the work performed; <br /> • Any difficulties or unexpected conditions encountered; <br /> 10 The title of this document will vary depending on the cleanup process. <br /> 37 <br />
