Activity Log: <br /> Date/Time Notable Activities <br /> 7/10/15 700 E Roth Rd., 200+/-transformer oil spill; met Muni at the site took pictures and <br /> interviewed Dan Herbst; Facility Manager. Mr. Herbst is also a manager of Wolverine a <br /> contractor for the Federal Government. Mr. Herbst said the release happened on July 6, <br /> 2015 at 10:32 PM due to a bushing failure which resulted in the release and fire that <br /> destroyed most of the substation. Herbst said samples were taken and clean-up will <br /> happen as soon as the power is stabilized. When asked why we were not notified until <br /> today, Mr. Herbst said the Fire Department said they would make the required notification. <br /> Also present were Desrie Blake and Don Yee, both Base Managers and Federal <br /> Employees. I took photos and reminded all present of the mandatory reporting <br /> requirements; requested initial and final samples. <br /> 8.
