PE 2546 BAKER <br /> 18 min <br /> SC 203 <br /> 05/16/2018 With Haza. <br /> 186 min E 54 <br /> PE 2546 BAKER <br /> At 8:24am I received a page from Esther about a spill. A semi truck struck <br /> 44 <br /> concrete causing the release of approximately 1 gallon of power steering fluid. <br /> The release was isolated to the pavement. No waterways or storm drains were <br /> impacted. The spill was located at Sharpe Army Depot past the entrance at 700 <br /> E. Roth Road. The spill was reported by Captain Keith Conner from the <br /> Lathrop/Manteca Fire Department. <br /> At 8:30 am, I called Captain Conner at (209)456-6525 and he told me that the <br /> spill was contained with kitty litter absorbent and there was not any active leaking. <br /> He gave me the name and address of the driver(Manjic Samra, 2318 Giannoni <br /> Way, Lodi, CA 95242) and the company name (Horizon Freight Systems INC). <br /> The fire department was no longer on the scene but the driver and police services <br /> was. Captain Conner stated that the spill was 1 gallon or less. <br /> At 8:52am I called Horizon Freight Systems INC at 1-800-480-6829 and they <br /> transferred me to Frank Koues in the claims department. I asked Frank when the <br /> clean up company would be arriving on site to clean up the absorbent. He stated <br /> that he would have to arrange for that and would call me back. He gave me the <br /> mailing address for the company (8777 Rockside Road, Cleveland, OH 44125). <br /> Frank called me back at 9:10am and told me that the driver would be taking the <br /> used absorbent. I told him that I would need disposal records for that and gave <br /> him my email address. <br /> At 9:45am Haza and I left the office to go to the spill site. We arrived at the site at <br /> 10:15 am. The San Joaquin County Sheriffs Department was on site. Officer <br /> Greg Mortenson gave me an incident number of 18-12387. The project manager <br /> of Sharpe Army Depot, Dan Herbst(209-814-8387), was on site. The driver of the <br /> truck was on site as well as the towing company (All Star INC Heavy Haul & <br /> Towing). The spill was contained with approximately a 5 gallon bucket amount of <br /> kitty litter. The spill area covered an approximate area of 5x4 feet. There was no <br /> active leaking and the roadway and surrounding area was inspected for any <br /> additional spills. No additional spills found. Garrett was called and he said that if <br /> the absorbent was not saturated it could be taken back and reused. The driver <br /> said that he will take the used absorbent for reuse. All Star INC Heavy Haul and <br /> Towing conducted the clean up after the truck was moved out of the way. The <br /> used absorbent was placed in two black bags. The used, unsaturated absorbent <br /> was taken by the driver to be reused. No hazardous waste was generated from <br /> the accident. We left the site at 10:55 am and arrived back at the office at 11:13 <br /> am. Haza and I did the Notice to Abate. At 12:32 pm I called Frank at Horizon <br /> Freight Systems INC to get the email address and to let him know the status of the <br /> spill and that the company would receive an invoice for our time spent on the spill. <br /> Frank also told me that the driver would be keeping the used absorbent in the <br /> truck for any future spills. I emailed the Notice to Abate to <br /> frankk@horizonfreightsystem.com and cc'd daniel.herbst@wolverinesvcs.com. <br /> Haza and I completed the photo log, Emergency Response Record and Prop 65 <br /> form and put together the er record paperwork. <br /> Page 1 of 1 <br />
