Resources Assigned: <br /> Name ICS Position Home Agency (and Unit) <br /> 7.Activity Log: <br /> Date/Time Notable Activities <br /> 2/19/14 Received a call from the Airport Operations Manager that a contractor for US Forest <br /> Service, First response Fire rescue, Hunout Retardant, and ICL was cleaning out some <br /> tanks and is letting all the run offs to go into the ground. Responded to the site with Robert <br /> lopez, Hazmat division. <br /> Arrived on scene at 5:15 PM and Talked to Sandra Eskridge. She said that the practice of <br /> dumping the waste onto the ground is a normal practice and happens all the time. <br /> Issued a notice to abate to the responsible party and took pictures. The workers present <br /> on site had no protective clothing on them and had very little knowledge of the clean up <br /> prcess. When asked where the PPE was, Ms Eskridge asked what PPE meant. <br /> While on site noticed that some of the liquid was going to a storm drain. I Informed Mr <br /> Turner to notify the City of Stockton Storm Water Division. <br /> Kept, Kasey Foley (PC) informed of the progress. At around 8:30pm received a call from <br /> Lolita Pearson (HR). She was inquiring about the spill and wanted to know the progress. <br /> Relayed to Lolita everything that had happened. <br /> 2/20/14 Called Eddie Miranda (CalOSHA) and made a complaint regarding the lack of training. <br /> Also, left a message for Dave Campodonica (US Forest Services) to call back. <br /> While on my way to the site received a call from Shannon Orn (Consultant). Had a <br /> conference call with Shannon (805 559 1904 cell) and Alfredo Aviles (909 957 2499). <br /> During the call I was informed that Hunout Retardant had been hired by US Forest services <br /> to provide fire retardant to the fire fighting planes. Shannon informed that the Phos-Chek <br /> is a mixture was 1: 5.5 ratio mixed with water and would not be hazardous. I informed him <br /> that the MSDS indicated that it is a RCRA hazardous waste. Shannon said that every two <br /> days the tanks were cleaned and the waste was dumped in the grassy area East of the <br /> tanks.
