The material safety data sheet used for the determination <br /> showed that the LC50 for the Fathead Minnow was tested to be 229 mg/L. A uniform hazardous waste manifest was not <br /> prepared for this waste. A generator shall prepare a Uniform Hazardous Waste Manifest on EPA Form 8700-22 prior to <br /> the transport of a hazardous waste for off-site transfer,treatment, storage or disposal. Ensure that a manifest is prepared <br /> each time hazardous waste is transported for off site transfer,treatment, storage or disposal. Immediately inform US <br /> Ecology Nevada of this incident and provide a statement detailing what they did with the waste. <br /> This is a Class I violation. <br /> OVERALL NOTES AND COMMENTS <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the <br /> corrective actions that have been or will be taken for each violation, and any supporting paperwork, by March 13, 2019. <br /> Starting September 1,2018, all in-office CERS help will be provided at EHD hourly rate($152).
