b. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s) being tested, repaired, or serviced. <br /> and <br /> c. Each person installing or testing any component must hold a certificate of training <br /> from the manufacturer of that component. <br /> 4. All components installed must be certified by an independent testing organization <br /> and bear the appropriate mark as required by the independent testing <br /> organization. <br /> 5. All monitoring equipment must be approved and listed in the State Water Resources <br /> Control Board's Local Guidance Letter 113:"List of Leak Detection Equipment and <br /> Methods for Underground Storage Tanks." <br /> 6. Complete and submit a copy of the Underground Storage Tank Certification of <br /> Installation/Modification for the 91 tank no later than 30 days after work is completed. <br /> 7. This permit expires 180 days from the approval date. <br /> 8. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work.
