Patterson Pass Road <br /> Tracy, CA 95376 <br /> Violation# Explanation Comment <br /> 3 One Facility form is filled and attached. <br /> Four forms for each tank is filled and attached. <br /> 4 Certification of financial form and CFO letter is attached <br /> 5 Monitoring form (2 pages) is filled and completed. <br /> Response form 2 pages) is filled and completed. <br /> 19 Overfill Prevention Mechanism <br /> Issue- Light bulb was bad. <br /> Correction— Light bulb was replaced. <br /> Verification -This was inspected to make sure that the <br /> mechanism is properly functioning. <br /> 12 PLLD Failure <br /> Issue - Reliable Petroleum technician detected that <br /> PLLD switch was malfunctioning. <br /> Correction -The PLLD switch was replaced by Reliable <br /> Peroleum. <br /> Verification -This was inspected to make sure that the <br /> leak detection mechanism is properly functioning. <br /> 32 Spill Bucket Failure <br /> Issue- Reliable Petroleum technician detected that <br /> Oring (washer)was bad. <br /> Correction -The Oring was replaced by Reliable <br /> Peroleum. <br /> Verification -This was inspected to make sure that the <br /> spill bucket mechanism is properly functioning. <br /> Certification <br /> Correction— Fill the Certification form. <br />
