Information and Instructions for Obtaining A Permanent California ID Number: <br /> Complete the application for a California Hazardous Waste Permanent ID Number and <br /> fax it to (916) 255-4703. <br /> 3. Certification of Financial Responsibility/Letter from Chief Financial Officer: Complete <br /> the form provided and make a copy for our records. This document is required to be <br /> submitted annually. <br /> 4. Underground Storage Tank Monitoring and Response Plan: Complete and submit a copy. <br /> Retain onsite for future reference by employees. <br /> 5. Hazardous Materials Management Plan: Contact the Office of Emergency Services, Mr. <br /> Dennis Fields, at 468-3969 for information on updating this plan. <br /> 6. Report#5021: Update all information and sign and date. Bring with all other forms to <br /> our accounting office to pay for transfer fees. <br /> 7. A signed "Designated UST Operator"must be submitted with current information. <br /> 8. Go to: http://www.waterboards.ca.gov/cwphome/ust/regulator}-,/index.litml - rg_s to obtain <br /> the current Statutes (Underground Storage of Hazardous Substances) and UST <br /> Regulations that you are required to have for your monitoring and response plan. <br /> 9.
