Box 388 • Stockton, CA 95201-0388 <br /> 2091468-3420 <br /> KATIE L HOWER <br /> CHEVRON PIPE LINE COMPANY <br /> w. 4000 EXEC PKWY BISHOP RANCH 8 <br /> SAN RAMON CA 94583 <br /> i-' RE: Nicholaw Property (APN: 238-080-02) SITE CODE: 505478 <br /> West Byron Road <br /> Tracy, CA., 95736 <br /> San .Joaquin County Public Health Services, Environmental Health Division (PHS-EHD) <br /> has recently reviewed the "Summary of Soil and Groundwater Assessment and <br /> Recommendation for Site Closure" as submitted by GeoMatrix Consultants and has the <br /> following comments. <br /> L <br /> PHS-EHD, in a recently modified agreement with the Central Valley Regional Water <br /> Quality Control Board (CVRWQCB), has added this site to the small list of Byron road <br /> LM sites that fail under the oversight this county's site mitigation unit. After review of the <br /> data, and the recent certification of the Dividend Site located just north of this site, it is <br /> the conclusion of PHS-EHD that no further investigative or remedial action is required at <br /> this time. <br /> Prior to certifying this site for closure, PHS-EHD will require the monitoring wells located <br /> on site to be properly abandoned under a permit issued by this agency. Please submit <br /> a completed boring/destruction permit application, $60 fee, and a short workplan that <br /> describes the method, material, and proposed date for the field work. <br /> As a special requirement for sites along this previous pipe line easement, the final issue <br /> of the Soils Management Plan (SMP) should be submitted to PHS-EHD for review and <br /> approval. Once the SMP has been approved by PHS-EHD, the well destruction <br /> procedure may proceed. Please stay in contact with Michael Infuma so that he may <br /> .. make an appointment with you to witness the well destructions. <br /> Once the wells have been properly destroyed, you will need to remit the balance due on <br /> your account for this site. You may wait until the final bill arrives approximately forty- <br /> five (45) days after the field work has been completed or you may contact Michael <br /> Infuma about a week after the field work to obtain the final balance. <br /> A ^n;<non of San COLM!%- HeAth Care=_nice, <br /> L <br />
