SAIC - Engineering and Infrastructure <br /> Health and Safety Plan for Chevron EMC Field Operations <br /> Job Name: Mountain House Development Site #5 Soil and Groundwater Investigation <br /> Emergency Reporting <br /> The SHSO will immediately report injuries or illnesses, vehicle accidents, releases (hazardous material, <br /> hazardous waste, or uncharacterized waste), inspections by regulatory agencies, and any incident that could <br /> reasonably have caused a significant injury or property damage(fire, catastrophic equipment failure, capsized <br /> heavy equipment, etc.). The SHSO will contact emergency response organizations (if needed)and the PM. The <br /> PM will notify the client(as appropriate), Division Manager, Program Manager, and ECHS Manager. If the PM is <br /> not available, the SHSO will contact the SAIC Regional Alternate Program Manager or other PMs, as necessary. <br /> If the SHSO is not available, the FM will make the aforementioned contacts. See Site Health and Safety <br /> Hierarchy on page 7. See the SAIC Incident Notification Contact List and the EMC Emergency Reporting <br /> contact protocol in Attachment 2 of this HASP (also posted in the Emergency Reporting tab section). See also <br /> EC&HS Procedures 4.1, Accident Reporting and 24, Regulatory Agency Inspections and Incident Reporting for <br /> details. <br /> Emergency Response Plan <br /> Emergency Evacuation <br /> Emergency evacuation of the site may be necessary under certain conditions. In the event that the site must be <br /> evacuated, the SHSO or other personnel will invoke a"Stop Work"authorization. One long blast from a portable <br /> air horn or vehicle horn will serve as the site emergency alarm. All work is to stop immediately. The SHSO or <br /> FM will call 911, the utility company(if necessary), safety personnel at adjacent facilities, and the PM. Workers <br /> will exit the site and proceed to the designated primary assembly point. The secondary assembly point shall be <br /> identified in the event that the primary assembly point is unsafe. The primary assembly point will be at the <br /> northeast intersection of Kelso Road and Byron Highway. The secondary assembly point will be located off of <br /> Byron Highway, slightly southeast and approximately 500 feet of proposed boring locations. Locations are <br /> found in Tab 3, Site Map. <br /> In the event that an alarm or other emergency action occurs at an adjacent facility, the SHSO will evaluate the <br /> emergency and reference the wind direction to determine whether or not the field team should assemble at the <br /> rally point(s)designate for the project. If the designated rally point(s)are deemed unsafe, the SHSO will have <br /> the field team meet at an alternate location and account for of all field personnel. When all field personnel have <br /> been assembled, the SHSO will lead the field team away from the emergency in a safe direction and conduct <br /> emergency notification protocols when the team has been relocated to a safe area. <br /> The SHSO or FM will use the SAIC Tailgate Safety Form to call roll and account for onsite personnel. If anyone <br /> is missing, that person's name will be forwarded to emergency response personnel. Emergency evacuation <br /> practice drills will be performed during the first week of work at the site and at least once every 3 to 6 months for <br /> each major task. Evacuation practice drills will be documented on the Emergency Evacuation Practice Drill <br /> Report. <br /> The SHSO will contact safety representatives and inquire about the emergency alarms and response protocols <br /> used at the facility prior to field mobilization. The SHSO will also verify that the emergency alarm to be used <br /> onsite does not conflict with an alarm used at adjacent facilities. The SHSO will present this information to the <br /> field team during daily tailgate safety briefings. The SHSO will include the telephone numbers of the safety <br /> departments at adjacent facilities in his/her list of emergency contacts in the event that an onsite emergency <br /> could affect an adjacent facility. <br /> Emergency Communication <br /> See Site Health and Safety Hierarchy Organization Chart included in this HASP.
