'Mar, 1 200$ 4; 52PM No. 2405 P. 1/1 pro <br /> .ACCESS AGREEMENT <br /> Subsurface Investigations -Monitoring Well Installation <br /> 521 East Oak Street, Stockton, California <br /> Advanced GeoEnvironmental, Inc. is hereby granted permission to perform a subsurface <br /> investigation at 521 East Oak Street, Stockton,California.Investigatory activities to be conducted <br /> will consist of drilling one 8.25-inch diameter soil boring to a depth of approximately 70-feet below <br /> surface grade to collect soil samples and install one ground water monitoring well.Upon completion <br /> of drilling,a two-inch diameter well casing will be placed through the center of the bore hole,and <br /> well construction materials added(i.e. sand,bentonite and grout)to fill the void space between the <br /> well casing and the borehole.An eight-inch diameter,traffic rated and flush mounted steel well box <br /> will be utilized to cover the well casing_The well box will be flush to the ground surface. After the <br /> installation of the well,the well will be sampled once every quarter. <br /> Prior to performance of any field operations,at least one week notification to Mr.Carlos Rodriguez will <br /> be given.Advancement activities will be completed in approximately 8 hours.The area utilized for well <br /> installation activities at 521 East Oak Street shal..l be returned to its original conditions after well <br /> installation activities are completed. <br /> All costs for performance ofthe subsurface investigation will be the sole responsibility of Stallworth Auto <br /> Sales located at 602 North California Street (Mr. Gary Stallworth), Mr_ Stallworth will be solely <br /> responsible for all costs and expenses for investigation associated with any contamination from the <br /> Stallwortb site. <br /> Copies of all testing and reporting for any investigation performed at 521 East Oak Street will be <br /> forwarded to Mr.Carlos Rodriguez. <br /> Mr.Carlos Rodriguez Date <br /> 128 Wilshire Cow <br /> Danville,CA,94526 <br />
