Include in the survey all municipal, domestic, <br /> industrial , agricultural and other wells located within a 2000-foot radius of the <br /> contaminant source area. Include in the survey a map with all located wells <br /> documented , and all pertinent data available about the wells including boring logs, as- <br /> built diagrams and their current use. Submit the completed well survey to PHS/EHD <br /> with the report of findings from the next phase of fieldwork from this site. <br /> Quarterly groundwater monitoring should continue at this site until further notice from <br /> PHS/EHD .
