The Site Safety Officer shall review with all personnel and subcontractors the equipment inspection checklist for company vehicles. The Site Safety Officer shall also confirm that adequate water is available at the drill rig for emergency decontamination and washing in case a worker is exposed to contaminated water or soil. Field vehicles shall park in an area that is a safe distance from operating equipment and vehicle traffic with the location approved by the Site Safety Officer. Care must be given when approaching heavy equipment. Personnel on foot should approach from a direction facing the operator and not a blind side. Eye-to-eye contact should be made before proceeding within the working limits of the operator and their heavy equipment. 2.6 OVERHEAD HAZARDS Overhead hazards include utility lines and trees. Prior to performing work requiring the operation of equipment above eye level, the evaluation of overhead hazards shall be made. Equipment shall be located f to maximize the clearance from overhead hazards (minimum 10 feet clearance). 2.7 UNDERGROUND UTILITIES Condor personnel have contacted Underground Service Alert prior to drilling activities. A private utility locator will also be used prior to investigation activities. The approximate location of underground utilities shall be marked on the existing ground surface. A minimurn of 3 feet clearance shall be given to marked utilities. If drilling near utilities is required, hand excavation and probing shall be performed in 6- inch intervals prior to drilling. 2.8 MOVING AND ROTATING MACHINERY Excavation activities involve the use of large, heavy, moving and rotating equipment. Loose fitting ,l clothing that can be caught or snagged should be avoided. Working gloves should be worn while working with moving metal parts and equipment. _ 3.0 PHYSICAL HAZARDS I_ 3.1 TRIP AND SLIP PROTECTION lGood housekeeping shall be maintained at the site to prevent trip and slip hazards . 3.2 HEAT STRESS Heat stress will be a hazard during hot weather and will be intensified when personnel are in protective clothing (if necessary) . To prevent heat stress, personnel will be required to take breaks as needed, and consume adequate quantities of liquid. 4.0 BIOLOGICAL HAZARDS 4.1 STINGING INSECTS Care should be exercised to avoid bees, yellow jackets, or other stinging or biting insects. u 4.2 ANIMAL High-topped boots should be worn when working in tall grass to protect against snake-bite. � J CONDOR
