We have concluded that due to site speci is impairments to normal well destruction, just filling <br /> the 4"casing with neat grout to the surface would suffice as an adeqi iate and necessary method for this wells' destruction. <br /> I realize that the EHD correspondence dated April 13, 2005 noted/rec uested a video log in the absence of well <br /> construction details, but obtaining these details would do nothing to c ange your recommended destruction techniques, <br /> since the limited space would not allow typical methods. Therefore, HD will NOT require a video log of this previously <br /> unknown well. The work plan is hereby approved by EHD and you may destroy the well as indicated. This is to serve as <br /> the official EHD work plan approval letter. <br /> Attached is the permit application form for such procedures. Just clic on it and print it out. Note, it is a larger form than the <br /> typical 8.5 x 11" standard paper. Be prepared to print on 8.5" x 14". Have a C-57 fill it out, sign it, and return to me with a <br /> check for$150, paybable to "SJCEHD". If you'd rather fill it out yourself, then have the C-57 send me a short <br /> "Authorzation to Sign" letter authorizing your signature on the application.
