California St. APN: 125380-16 <br /> Stockton, CA 95204 SWRCB-CUF#: 011775 <br /> WORKPLAN: Well Abandonment Plan (Revised) <br /> DATED: July 13, 2011 <br /> PREPARED BY: Foothill Engineering - Michael Vlach, PE. <br /> STATUS: APPROVED with conditions noted below. <br /> ADEQUATE AND NECESSARY ELEMENTS OFI THE WORKPLAN Health & Safety <br /> Cade (H&S) Section 25296.10(c)(3)1: <br /> • In preparation for providing the No Further Action (NFA) status letter, destroy 17 <br /> monitoring wells by removing the contents and 3 monitoring wells by pressure <br /> grouting. <br /> • Submit a report of field activities upon completion of the field work and include all <br /> manifests of investigation derived waste. <br /> COMMENTS AND CONDITIONS: <br /> • Wells proposed for pressure grouting shall be pressure grouted before any traffic <br /> box demolition or `mushroom cap' placement to ensure the casing is not <br /> obstructed or damaged. After the casing has held liquid grout pressure of 25 psi <br /> for 10-minutes, the mushroom cap work may proceed. <br /> • The EHD has been informed that the 20 well destruction processes will be <br /> performed over a 12-month to 18-month period. This extended time period <br /> portion of the proposal is inadequate and unacceptable. Although the State <br /> Water Resources Control Board (SWRCB) Clean Up Fund (CUF) is limiting <br /> reimbursement amounts due to financial problems, the EHD has been assured <br /> that monies are available to eligible responsible parties for well destructions for <br /> sites preparing for NFA. The EHD requires you to submit a Well Permit <br /> Application Form and fee for each parcel your wells are located on. A copy of a <br /> signed access agreement is required for off -site private parcels not under your <br /> ownership and a City of Stockton encroachment permit is required for well <br /> destructions located in the City right of wary. The City permit and the private <br /> access agreement copies and the EHD application forms and fees are due <br /> at the EHD by September 30, 2011. All wells are to be destroyed before <br /> November 30, 2011 and the report of the field activities is due on <br /> Geotracker for EHD review on January 210, 2012. <br /> j <br />
