If anything has changed on the Master File Record (MFR) form, then you must submit a new MFR form <br /> when you submit the permit application and check for$1,000.00. <br /> Our field inspector, Johnny Yoakum, is very busy so it is important that you contact Johnny at 209.468.3147 to confirm his <br /> availability, once the field work has been scheduled. <br /> I did check with Adrienne Ellsaesser concerning the destruction of MW-1D and Adrienne's instructions in her email dated <br /> 11 July 2012 still apply; however, things have changed concerning the well destructions of nested wells VW-1A, VW-1 EI, <br /> VW-2A and VW-26.
