Box 2009 <br /> Stockton, CA 95201 <br /> RE: St. Joseph's Medical Center of Stockton, Cemetery Lane and McCloud Avenues, <br /> Stockton, California <br /> I _ - <br /> Dear Mr._Infurna <br /> In response to the telephone conversation between yourself and Mr. Greg Stahl onNovember <br /> 23, 1992, this correspondence proposes the abandonment of well MW1, as requested for closure <br /> of,the above referenced site. <br /> The following°activities will be conducted in accordance with San Joaquin County Public Health <br /> Services, Environmental Health Division, Revised Well Standards (Rules &Regulations) dated <br /> August 1992. Well destruction will be conducted under the supervision of a California <br /> Registered Geologist by a C-57 licensed-contractor. Well destruction will include the following <br /> tasks: <br /> (1) the well will be soundedimmediately prior to destruction to ensure no obstructions will <br /> interfere with- well destruction,.if an obstruction is found it will be removed prior to <br /> destruction; <br /> (2) a hole will be excavated around'the top of the well; and-the well casing will be cut off <br /> to 2.5 feet below grade <br /> (3) the well will be pressure grouted, with a tremmie system, from the bottom of the well <br /> (total depth = 76 feet) to.the surface.
