City of Lathrop `aTyRop Public Works Department 16775 Howland Road Lathrop, California 95330 E (209) 858-2860, Ext. 328sa Fax (209) 858-1023 ENCROACHMENT PERMIT NO. 441 SUPPLEMENTAL CONDITIONS APRIL 16,2002 The work set forth in the above referenced submittal, a copy of which is attached hereto, is authorized, subject to the following conditions: 1. The applicant acknowledges that this is a revocable permit and the city may revoke this permit at any time. Should the wells interfere with any road improvement projects or become a safety hazard, applicant agrees to relocate at their cost. 2. All work shall conform to the current City of Lathrop Design and Construction Standards. a. The Public Works Department shall be contacted 48 hours prior to construction start up to schedule an inspection and/or a pre job meeting. b. All asphalt repair work shall be at the direction of the City Inspector. 3. The contractor shall submit a traffic control plan for approval of the City Engineer. 4. The contractor shall notify USA of any effected agencies not subscribing to USA with proper notice, prior to construction of the wells. (The City Engineer shall have the final decision on locations of the wells.) 5. The applicant shall provide the City of Lathrop Public Works Department with a copy of the County Environmental Health Permit. 6. The applicant shall post a cash bond in the amount of$2,500.00 per well to insure wells are maintained and abated. a. (3)wells @ $2,500.00 each=$7,500.00 NOTE: Should additional groundwater monitoring wells need to be installed, additional bonding will be required. 7.
