City of Lathrop `aTyRop <br /> Public Works Department <br /> 16775 Howland Road <br /> Lathrop, California 95330 E <br /> (209) 858-2860, Ext. 328sa <br /> Fax (209) 858-1023 <br /> ENCROACHMENT PERMIT NO. 441 <br /> SUPPLEMENTAL CONDITIONS <br /> APRIL 16,2002 <br /> The work set forth in the above referenced submittal, a copy of which is attached hereto, is <br /> authorized, subject to the following conditions: <br /> 1. The applicant acknowledges that this is a revocable permit and the city may revoke <br /> this permit at any time. Should the wells interfere with any road improvement <br /> projects or become a safety hazard, applicant agrees to relocate at their cost. <br /> 2. All work shall conform to the current City of Lathrop Design and Construction <br /> Standards. <br /> a. The Public Works Department shall be contacted 48 hours prior to construction <br /> start up to schedule an inspection and/or a pre job meeting. <br /> b. All asphalt repair work shall be at the direction of the City Inspector. <br /> 3. The contractor shall submit a traffic control plan for approval of the City Engineer. <br /> 4. The contractor shall notify USA of any effected agencies not subscribing to USA <br /> with proper notice, prior to construction of the wells. (The City Engineer shall have <br /> the final decision on locations of the wells.) <br /> 5. The applicant shall provide the City of Lathrop Public Works Department with a <br /> copy of the County Environmental Health Permit. <br /> 6. The applicant shall post a cash bond in the amount of$2,500.00 per well to insure <br /> wells are maintained and abated. <br /> a. (3)wells @ $2,500.00 each=$7,500.00 <br /> NOTE: Should additional groundwater monitoring wells need to be installed, <br /> additional bonding will be required. <br /> 7.
