(SGI) has prepared this well <br /> destruction Work Plan for the Flag City Chevron located at 6421 Capitol Road, Lodi, California, <br /> referred to as Emergency Abandonment and Recalcitrant (EAR) Site No. R10-130 (Site). This <br /> Work Plan has been prepared in response to CRWQCB correspondence entitled Workplan <br /> Approval Granted to Decommission Wells Flag City Chevron, 6421 Capitol Avenue, Lodi, San <br /> Joaquin County Cleanup and Abatement Order No. 5-00-700 (LUST Case #390782), dated <br /> March 25, 2008 (Appendix A). The Work Plan details the proposed scope of work and <br /> methodology to destroy all continuous multi-band tubing (CMT)wells associated with the Site. <br /> BACKGROUND <br /> The Site is located at the northeast intersection of Capitol Avenue and Thornton Road, <br /> approximately 500 feet south of California State Highway 12 and approximately five miles west <br /> of Lodi, California. A Site location map and Site plan are included in Appendix B. Background <br /> information was obtained from the Advanced GeoEnvironmental (AGE) Closure Summary <br /> Report—August 2005, dated August 31, 2005. <br /> The property is currently owned by the Bokides Family Trust (Mr. Richard Bokides). Mr. <br /> Bokides has stated that he leased the property from Mr. Cecil Dillon between 1984 and 1994, <br /> and he purchased the property circa 1994. The property has been utilized as a retail fueling <br /> station from approximately 1984 to present day. Prior to 1984, the property was utilized as a <br /> saddle shop. Mr. Bokides was part of the Flag City Chevron daily business operations between <br /> 1984 and 1997 however, the underground storage tanks (UST's) have been operated by Flag <br /> City First Investment (Nick and Karen Dhingsa) from 1997 to present day. The Flag City <br /> Chevron property currently consists of one building structure (mini-mart), six fuel dispensers, <br /> three 12,000-gallon gasoline USTs and one 12,000-gallon diesel UST.
