Likewise, if push and pressure grouting are unsuccessful, use of the explosive well <br /> decommissioning method would also require that any post detonation cavity in the boring <br /> be tremied or filled with grout per the SJCEHD inspector's instructions. <br /> 4. At no time shall the use of explosive charges create a danger to human health, to <br /> adjacent monitoring wells, or to buried utilities. All safety protocols shall be enforced and <br /> a barrier exclusion zone shall be erected to prevent unauthorized access to the well <br /> during destruction. All explosive devices will be handled, stored and licensed per <br /> applicable State and Federal regulations. Fueling operations will be suspended within <br /> one hundred feet of the CMT well while charges are being placed and the wells <br /> detonated. The Flag City Fire Department is receiving a copy of this letter and will <br /> reserve the right to suspend operations if deemed unsafe by that agency. And, you are <br /> to notify me, and receive approval by myself or other applicable Regional Board staff, at <br /> least 5 working days prior to implementation of the line charge method. <br /> 5. Signed copies of SJCEHD well destruction permits and a schedule to decommission the <br /> wells are due at this office by 15 April 2008. <br /> 6. Submit a report of the well activities by 1 July 2008, which describes how the wells were <br /> properly decommissioned, and include copies of signed agreements that the remaining <br /> wells have been transferred to other parties for their investigations, and that the receiving <br /> parties agree to accept responsibility for the wells, including proper maintenance and <br /> decommissioning (when directed to do so by this agency).
