INTRODUCTION AND SCOPE OF WORK <br /> At the request of Mr Richard Bokides,Advanced GeoEnvironmental, Inc (AGE) has prepared this <br /> quarterly report for 6421 Capitol Avenue (the site), Lodi, California The report documents <br /> procedures and results for remediation and subsurface investigations conducted at the site during <br /> October,November and December 2000(fourth quarter) The enclosed quarterly report was prepared <br /> as required by Central Valley Regional Water Quality Control Board (RWQCB) Cleanup and <br /> Abatement Order No 5-00-700, dated 01 March 2000 The site setting is illustrated on Figure 1 Site <br /> structures and monitoring well locations are depicted on Figure 2 <br /> The objectives of fourth quarter investigations were to characterize, assess and monitor dissolved <br /> petroleum hydrocarbons and methyl tert butyl ether (MTBE) concentrations in ground water, to <br /> characterize ground water flow direction and gradient at the site and to implement a ground water <br /> remediation system at the site This report is prepared in accordance with guidelines established by <br /> the California Regional Water Quality Control Board (CRWQCB) - Central Valley Division for <br /> investigation of underground storage tank (UST) sites Background information and a description <br /> of prior investigations at the site were included in AGE's Final Remediation Plan, dated 05 October <br /> I � 2000 <br /> The following is a brief summary of fourth quarter site assessment and remediation activities <br /> performed at the site <br /> • 01 October through 30 October 2000 - Continued Interim Remediation at EW-1, <br /> • 09 through 17 October 2000 - Installation of ground water extraction wells (EW-4, EW-5, <br /> EW-6, EW-7, EW-8, EW-9 and EW-10), <br /> I • 26 through 31 October 2000- Installation of ground water extraction pumps at EW-2, EW-4, <br /> EW-5, EW-6, EW-7, EW-8, EW-9 and EW-10, <br /> a 18 October through 10 November 2000 - Installation of ground water pump and treat <br /> remediation system <br /> I • 09 through 20 November 2000 - Ground water pump and treat `start-up' period <br /> • 22 November through 31 December 2000 -Discharge petroleum-treated water into Coldam <br /> Ditch <br /> • 13 and 14 December 2000 - Fourth Quarter 2000 ground water monitoring event <br /> i <br /> • <br /> IAdvanced GcoEnvoronmentsl,Inc <br />
