• Quarterly Report - Second Quarter 2004 <br /> FLAG CITY CHEVRON <br /> 6421 Capitol Avenue, Lodi, California <br /> 1.0. INTRODUCTION AND SCOPE OF WORK <br /> At the request of Mr Richard Bokides,Advanced GeoEnvironmental, Inc (AGE)has prepared this <br /> quarterly report for 6421 Capitol Avenue (the site), Lodi, California The report documents <br /> procedures and results for ground water monitoring and remediation performed at the site between <br /> 16 March 2004 and 15 June 2004 (second quarter) The site setting is illustrated on Figure 1 Site <br /> structures and monitoring well locations are depicted on Figure 2 <br /> The enclosed report was prepared as required by California Regional Water Quality Control Board- <br /> Central Valley Region (RWQCB) Cleanup and Abatement Order No 5-00-700, dated 01 March <br /> 2000,and was prepared in accordance with guidelines established by the RWQCB for investigation <br /> of underground storage tank (UST) sites Background information and a description of prior <br /> investigations at the site were included in AGE's Final Remediation Plan, dated 31 August 2000 <br /> The objectives the of second quarter investigations were to characterize, assess and monitor <br /> dissolved petroleum hydrocarbons and methyl tert-butyl ether (MTBE) concentrations in ground <br /> water,to characterize ground water flow direction and gradient and to continue ground water <br /> pump-and-treat remediation at the site The following is a brief summary of second quarter site assessment <br /> and remediation activities <br /> Between 16 March and 22 April 2004 - The pump-and-treat remediation system was <br /> operational,utilizing extraction wells EW-4,EW-5,EW-8 and EW-10 to treat approximately <br /> 674,029 gallons of Flag City Chevron MTBE-impacted ground water,however,between 22 <br /> April and 28 Apnl 2004, the pump-and-treat remediation system was non-operational due <br /> to a mechanical malfunction with the process pump A new process pump was installed on <br /> 28 April 2004 and the pump-and-treat system restarted <br /> 28 April 2004 - Due to elevated MTBE concentrations detected at EW-7 during the first <br /> quarter 2004 ground water monitoring event(17 March 2004),the submersible pump utilized <br /> at extraction well EW-9 was removed and reinstalled in well EW-7 After transferring the <br /> submersible pump from EW-9 to EW-7, extraction well EW-7 was restarted and brought <br /> back on-line with extraction wells EW-4, EW-5, EW-8 and EW-10 Currently, extraction <br /> wells EW-6 and EW-9 are off-line and do not have submersible pumps <br /> Between 28 April and 15 rune 2004 - Ground water pump-and-treat remediation was <br /> continued at the site utilizing extraction wells EW-4,EW-5,EW-7,EW-8 and EW-10,a total <br /> of approximately 53 8,2 63 gallons of treated Flag City Chevron ground water was discharged <br /> into a conveyance ditch located approximately 450 feet south of the site <br /> 14 April, 13 May and 15 June 2004-Monthly monitoring events at the ground water pump- <br /> and-treat remediation system <br /> Advanced GeoEnvironmental,Inc <br />
