AGREEMENT FOR WATER SERVICE FROM COUNTY SERVICE AREA 31 <br /> (ASSESSOR'S PARCEL NO. 055-150-25/14659 NORTH THORNTON ROAD, <br /> LODI, CALIFORNIA) <br /> Dear Mr Bokides <br /> County Service Area 31 will provide water service to Rocky's Restaurant(Assessor's Parcel <br /> No 055-150-25)subject to the following terms and conditions; <br /> Rocky's Restaurant agrees to. <br /> 1 Apply for an Out-of-Agency Service Agreement through the Local Agency Formation <br /> ICommission, and pay all Local Agency Formation Commission processing fees. <br /> 2 Pay for any appropriate buy-int fees and an Area of Benefit fee. The buy-m fee is <br /> $2,150 61 based on a 0.980-acre parcel Payment of the Area of Benefit fee for Well <br /> No 3 will be required when the rate is established_ The estimated fee of$15,000 shall <br /> be prepaid to the County of San Joaquin prior to connection to the water system <br /> I3 Annex to County Service Area 31, when sewer capacity is available, for sewer service, <br /> and pay the annexation fee in effect at the time of annexation. The estimated fee of <br /> $25,000 shall be prepaid to the County of San Joaquin prior to connection to the water <br /> system <br /> I 4 Pay the normal and customary service charge as adopted each fiscal year by County of <br /> San Joaquin,based on meter size and water consumption. For Fiscal Year 2001-02, <br /> the rate is a fixed monthly rate based on meter size plus$1.88 per 1,000 gallons used <br /> 5 Install a water meter with size and location approved by the County <br /> 6 Abandon and seal the existing well under a permit from the County Environmental <br /> Health Department <br />
