INTRODUCTION AND SCOPE OF WORK <br /> At the request of Mr Richard Bokides of the Bokides Family Limited Partnership, Advanced <br /> GeoEnvironmental, Inc (AGE), has prepared this Problem Assessment Report (PAR) for 6421 <br /> Capitol Avenue (site), Lodi, California The location of the site is illustrated in Figure 1 A plan of <br /> the site is illustrated in Figure 2 <br /> The scope of work for this PAR is to summarize the preliminary investigation and evaluation of the <br /> subsurface release on the site, to describe the lateral and vertical extent of fuel components impact <br /> to soil and ground water and to evaluate the feasibility and costs of several remedial action <br /> alternatives for petroleum-hydrocarbon impact to soil and ground water at the Flag City Chevron <br /> site This PAR was prepared in accordance with guidelines established by the Regional Water <br /> Quality Control Board - Central Valley Region (RWQCB-CVR) for preliminary investigation and <br /> evaluation of underground storage tank (UST) sites <br /> 2.0. BACKGROUND <br /> The site is located at the northeast intersection of Capitol Avenue and Thornton Road,approximately <br /> 500 feet south of California State Highway 12 and approximately 5 miles west of Lodi, California <br /> (Figure 1) The site is located in the northeast quarter of Section 14, Township 3 North, Range 5 <br /> East, Mount Diablo Base and Meridian (Temmous Quadrangle, 7 5-Minute USGS Topographic <br /> Senes, 1978, photo revised 1993) <br /> 2 1 SITE LOCATION AND DESCRIPTION <br /> The property is currently utilized as a Chevron fueling facility and a mini-market The site consists <br /> of one building structure, six fuel dispensers, three 12,000-gallon gasoline USTs and one 12,000- <br /> gallon diesel UST (Figure 2) <br /> The property west of the site(Three B's Truck Plaza- 14749 North Thornton Road)is approximately <br /> 200 feet west-northwest of the Flag City Chevron site and is currently utilized as a mini-mart and <br /> truck stop (Figure 3) <br /> 2 2 UNDERGROUND STORAGE TANK REMOVAL FLAG CITY CHEVRON <br /> On 31 March 1995, three 12,000-gallon gasoline USTs and one 12,000-gallon diesel UST were <br /> replaced at the site During excavation activities, ground water was encountered in the UST cavity <br /> Advanced GeoEn%Eronmental,Inc <br />
