INTRODUCTION <br /> At the request of lvIr Richard Bokides of Flag City Chevron, ,advanced GeoEnvironnmental, Inc <br /> (AGE) has prepared this Kork plan addendum for the drilling of five soil borings utilizing cone <br /> penetrometer testing (CPT) technology and for the installation of five groups of clustered ground <br /> water monitoring wells at 5421 Capitol Avenue, Lodi, California (site) The location of the site is <br /> illustrated on Figure 1 A plan of the site is illustrated on Figure 2 <br /> On 01 March 2000, the Central Valley Regional Water Quality Control Board (RWQCB)directed <br /> by Cleanup and Abatement Order No 5-00-700 that the horizontal and vertical extent of methyl <br /> tertiary butyl ether(MTBE)-impact be defined both on and off the site (page 4, section 6C) This <br /> work plan addendum has been prepared in accordance with San Joaquin County Public Health <br /> Services-Enviroimiental Health Division(PHS-EHD)and RWQCB guidelines for the investigation <br /> of underground storage tanks (UST) sites <br /> This work plan,provides a description of the scope of work and procedures:for five CPT soil bonnns <br /> and for installation of five groups of clustered ground water monitoring wells <br /> Z.O. BACKGROUND <br /> The site is located at the northeast intersection of Capitol Avenue and Thornton Road,approximately <br /> 500 feet south of California State Highway 12 and approximately 5 miles west of Lodi, California <br /> (Figure 1) The site is located in the northeast quarter of Section lh, Township 3 North, Range 5 <br /> East, Mount Diablo Base and Mendian (Tenrunous Quadrangle, 7 5-Minute USGS Topographic <br /> Series, 1978, photo revised 1993) <br /> 2 1 SITE DESCRIPTION <br /> The property is currently utilized as a Chevrou fueling facility and a mini-market The facility <br /> consists of one building structure, a canopied structure for the fuel dispensing area, and four 12,000- <br /> gallon petroleum USTs <br /> 2 2 UNDERGROUND STORAGE TANK REMOVAL <br /> On 31 March 1995, four 12,000-gallon gasoline USTs and one 12,000-gallon diesel UST were <br /> replaced at the site During excavation activities, ground water was encountered in the UST cavity <br /> at approxi nately 8 feet below site grade (bsg) Soil and ground water samples were collected from <br /> Advewred GenYnvifonmentfkL LUC <br />
