Manifest numbers 005523869SKS,006100725SKS, 006192365SKS, and 006520364SKS were found without a signed off copy from the destination facility. Manifest number 005523868SKS was not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter,the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. By March 18, 2019, submit to the EHD a copy of the signed off copy of the manifests or a copy of the exception report submitted to DTSC. This is a minor violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by March 18, 2019. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152). To schedule an appointment, please call (209)468-3420. Waste Streams Observed: oily absorbent, oil/water separator sludge, aerosol cans, parts washer solution, used antifreeze, used oil, brake lathe shavings, shop rags, and drained used oil filters. Note: Contingency Plan lists Mark Morgan, who no longer works at Lodi Honda, and Miguel Gallo as an emergency coordinators. Please update the contingency plan with current emergency coordinators. AFTER THE COMPLIANCE DATE, EHD WILL BILL FOR ALL TIME AND ACTIVITIES ASSOCIATED WITH BRINGING THIS FACILITY BACK INTO COMPLIANCE. THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE.
