COMPLAINANT ALLEGES AT LEAST TEN FLIES AT COMPLAINANTS TABLE, <br /> COMPLAINANT KILLED FOUR." <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Spoke with the Manager on duty regarding the fly complaint. She was not aware of the complaint. <br /> Observations this date: <br /> 1)The fly fan above the east sliding doors is turned off at arrival. Employee turned on the fly fan. It seems to be in need of <br /> repair(very noisy). Fly fans at south entrance door is turned off at arrival. Inspector turned on and it is functional. The fly fan at <br /> the drive thru is on. <br /> 2) Numerous flies were observed at several of the dining room tables, at the trash can (east side dining area)and in the <br /> men's restroom. <br /> 3)A few fruit flies were observed at the cabinet in between the Freestyle machines. <br /> 4) Facility uses Ecolab for pest control.The last service date was 6-26-17. <br /> 5) Facility has 4 UV light with glue board fly catcher units. One is located at the trash can at the east side dining area,two are <br /> located in the south dining room area and one is located in the kitchen. The UV light is not"On"at the kitchen unit. <br /> Corrective Action Required: <br /> 1)Take immediate action to remove flies within the facility.
