N.JOAQUIN Environmental Health Department <br /> COUNTY <br /> t;reocnt?ss grows Akers. <br /> Time In: 1:20 pm <br /> Time Out: 1:50 pm <br /> Food Program Complaint Inspection Report <br /> Name of Facility: SAMS PIZZA&MORE Date: 04/18/2018 <br /> Address: 1854 COUNTRY CLUB BLVD, STOCKTON 95204 <br /> Owner/Operator: HAK,VANNARA Telephone: (209)463-0210 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00046326 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT FOUND A COCKROACH IN PIZZA AND SAW SOME ON THE WALL NEAR THE SODA MACHINE. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> #21 Hot and Cold Potable Water Not Available <br /> OBSERVATIONS:Hand sink lacks hot water(was turned off).Turn back on, immediately. <br /> CALCODE DESCRIPTION:An adequate,protected,pressurized,potable supply of hot water and cold water shall be provided at all times. <br /> (113953(c), 114099.2(b) 114101(a), 114189, 114192, 114192.1, 114195) <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Spoke to owners spouse who was aware of this complaint. She stated that customer send them a letter and a picture of the <br /> contaminated pizza. She stated that picture was not very clear(all black)and that they could not see a roach. According to <br /> the customer they bought 3 pizzas. <br /> Observation: Inspected entire restaurant.
