SAN JOAQU I N Environmental Health Department <br /> CO LJ N T Y <br /> Time In: 9:40 am <br /> Time Out: 10:20 am <br /> Food Program Complaint Inspection Report <br /> Name of Facility: MUSD-GREAT VALLEY ELEMENTARY SCHOOL Date: 05/29/2018 <br /> Address: 4223 MCDOUGALD BLVD, STOCKTON 95206 <br /> Owner/Operator: MANTECA UNIFIED SCHOOL DISTRICT Telephone: (209)825-3200 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00046654 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> C00046654 states: "COCKROACHES IN CLASSROOM AND IN CAFETERIA. ON MAY 17, 2018, COMPLAINANT'S <br /> CHILD HAD COCKROACH IN SCHOOL FOOD." <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Upon arrival, spoke with the school principal. She is aware of a portion of the complaint(not aware of the portion alleging <br /> cockroaches in the student's food). <br /> School is currently not in session (school out for the summer on 5-25-18) <br /> School kitchen and cafeteria inspected (custodian unable to locate the key for the dry goods storage room). Upon inspection, <br /> four dead/dying cockroaches were observed in the kitchen (floor sink by water heater,floor sink by hood, and on the floor by <br /> the walk-in)and three dead/dying cockroaches were observed in the cafeteria(on the floor).All dead/dying cockroaches were <br /> either American or Oriental (no German). <br /> Facility uses Paramount Pest Control.A work order was submitted 5/8/18. Pest Control there at 5/10/18 at 6pm to treat areas. <br /> Another work order was submitted 5/14/18 and the facility was due to be sprayed 5/16/18 at 6pm. <br /> Please provide copies of pest control invoices with service dates from 5-8-18 to current. Submit via fax or e-mail to Kadeanne <br /> Linhares(klinhares@sjcehd.com). <br /> Clean up all dead insects. Clean and sanitize the areas.
