Contact the local fire department and state fire marshal for applicable permits and <br /> requirements. <br /> 2. Any changes in information or scope of work must be submitted to EHD prior to <br /> commencing work. <br /> 3. Complete the Masterfile Record Information and submit to this office. A copy is <br /> enclosed. <br /> 4. Provide the summary report of initial emergency response including the waste disposal <br /> manifest and receipt of recovered crude oil being recycled. <br /> 5. Obtain a well permit from this office and notify this office at least 48 hours prior to the <br /> proposed schedule work dates. Contact Harlin Knoll, Senior R.E.H.S. at (209) 468-3442 <br /> to arrange appointment. A copy of well permit application is enclosed. <br /> 6. All EHD staff time is charged at the rate of$93/hour. The response party of this project <br /> is currently known to be Mr. Charlie Spatafore.
