An additional billing party information form will need to be completed and charges for oversight and review of the <br /> report will be sent to this new responsible party. Increasing contamination in the groundwater may require additional <br /> investigation or remediation as determined by staff evaluation. <br /> The second option is to destroy the wells under an approved workplan and permit(s)issued by PHS-EHD.The workplan is <br /> to be brief and concise and is to describe the procedures, materials, and schedule for the field work. As requested by your <br /> consultant, Parsons Engineering,the well destruction section of San Joaquin County's Well Standards have already been <br /> forwarded to Chris Torell for his reference. <br /> Well destruction permits are issued to the parcel where the wells are located after the$60 fee per permit is paid and the <br /> workplan is approved. Wells located in the public right of way(ROW)will require additional permit(s)from the agency in <br /> charge of the property. MW-4 is located in the City of Stockton ROW and Mr.Jason Ender should be contacted for <br /> permitting requirements at(209)937-8349. After the City of Stockton has approved the proposal for MW-4's destruction, <br /> PHS-EHD will issue this well's destruction permit. <br /> After the wells are properly destroyed or a monitoring plan and schedule is approved,the"No Further Action Letter"can be <br /> issued.
