PAUL N <br /> DALLAS, TX 75266-0362 <br /> UNDERGROUND STORAGE TANK CLEANUP FUND PROGRAM, NOTICE OF PERMIT WAIVER AND CLAIM <br /> ACCEPTANCE: CLAIM NUMBER 011543; FOR SITE ADDRESS: 121 CENTER ST S, STOCKTON <br /> Your claim has been accepted for placement on the Priority List in Priority Class "D". <br /> Under the amended provisions of Section 25299.57 of the Health and Safety Code (H&SC), the <br /> State Board has granted your request for a waiver for the permit requirement as a condition for <br /> eligibility to the Fund. It is important to note that when a claimant failed to apply for or obtain <br /> the permits required pursuant to Chapter 6.7, Division 20, of the H&SC, by January 1, 1990, and <br /> the State Board grants a waiver pursuant to Section 2811(a)(2)(B) of the Underground Storage <br /> Tank Cleanup Fund Regulations, the claimant's level of financial responsibility (deductible) is <br /> twice the amount otherwise required. In this case, you will be responsible for the first$20„000 <br /> of eligible corrective action costs before the Fund coverage begins. <br /> After adoption of the Priority List, staff will review, verify, and process applications based on <br /> their priority and rank within a priority class. During this detailed review, staff may request <br /> additional information needed to verify eligibility. Once review of the application is complete <br /> and the claim is determined to be valid, a Letter of Commitment will be issued obligating funds <br /> toward cleanup. <br /> if, during the detailed review, it is determined that the claim application contained fraudulent <br /> information or misrepresentation making the claim unacceptable or ineligible, your claim may <br /> be rejected. In such event, you will be issued a Notice of Intended Removal from the Priority <br /> List, informed of the grounds for the proposed removal of the claim, and provided an <br /> opportunity to correct any deficiencies which are the basis for the proposed removal. <br /> If you have any questions, please contact me at (916) 227-4334. <br /> Sincerely, <br /> Annabel L.
